La tradizione assegna le origini del santuario ai primi tempi apostolici e, quindi, con la nascita del Cristianesimo. Da Petronio Arbitro (nel “Quo vadis”) siamo venuti a conoscenza che ai piedi della grotta, nelle vicinanze della tomba di Virgilio, esisteva un tempio dedicato a Lampsaco, dove di notte si celebravano lubrici riti pagani al ritmo di canti fescennini. (Tali canti costituirebbero i primordi di quella che poi diventerà la canzone napoletana). Dopo che S. Pietro, nel 44 d.c., portò a Napoli i valori spirituali del Cristianesimo questi, dai primi convertiti dalle sue predicazioni, vennero posti sotto la protezione di Maria nella sua espressione biblica: la Donna che schiaccia la testa del serpente. Il tempio di Lampsaco venne abbattuto e sulle sue rovine venne costruita una cappella dedicata a S. Maria dell’Idria (o del serpente). La leggenda vuole che la Vergine nel 1356, l’8 settembre, apparve in sogno a tre persone contemporaneamente: a un tal Benedetto, prete di S. M. a Cappella, a Maria di Durazzo monaca di Castel dell’Ovo e a un tal Pietro, un eremita, chiedendo loro di costruirLe un tempio ai piedi dell’antica grotta in onore di Dio e suo. Si vuole ancora che proprio durante gli scavi per l’erezione di tale tempio venisse ritrovata la statua della Madre di Dio che ancor oggi veneriamo.
